Anderson Family Care, a new startup founded by former leaders of hospice and home health companies, has launched two brands to improve access to end-of-life and palliative care: Anderson Hospice, a Nashville-based hospice program, and Counsel Care, a value-based care navigation platform.
Both brands launched in late September. Founders previously led organizations including Amedisys, Contessa Health, and WellBe Senior Medical.
Anderson Hospice operates in Nashville, Tennessee; Atlanta and northern Georgia through a partnership with Willowbrook Hospice; and Tucson, Arizona through Copper State Hospice. The company said it is seeking expansion into up to 10 additional markets and invests in small, independent, privately owned hospice programs to help them grow, providing assistance in compliance, technology, and other resources.
Counsel Care is designed for primary care providers and risk-bearing entities and uses a proprietary data analytics algorithm called HospiceIQ to identify patients earlier in their illness trajectories. Its benefits counselors spend an average of three to four hours with each patient discussing goals and end-of-life care options at no cost to the patient.
Counsel Care is active in Arizona, California, Florida, Georgia, Nevada, Oregon, and Texas and works with risk-bearing primary care groups, Medicare Advantage organizations, and accountable care organizations, with plans to expand nationwide.
