Team Select Home Care invested $6 million in a proprietary predictive AI platform called CareSight AI, trained on more than a decade of patient records, to catch subtle clinical decline and keep medically fragile patients out of the hospital.
The platform now covers 100% of the company's patient census, and Team Select reported hospitalization rates dropped 14.9% year-over-year between April and August 2026 compared with the same period in 2025, building on an already-low baseline from a seven-state Medicaid claims study.
CareSight AI, launched in 2025, monitors up to eight vital signs in real time along with nurse notes and changes in condition. Development moved quickly across disease-specific models, and the company added a second, patient-specific model to reduce false positives by checking alerts against each patient's historical baseline.
President and CEO Fred Johnson said the outcomes data is being used to reshape payer negotiations and pursue more value-based contracts, where rate increases would be tied to delivering on hospitalization reduction and total cost-of-care reduction.
Team Select, based in Phoenix, Arizona, serves medically complex pediatric patients and seniors in the home across 16 states and has been backed by private equity firm Court Square Capital Partners since 2023.
